CONCEPT :

The decrease in functioning and ability to bind to its substrates, which are the acetyl L carnitine and coenzyme A acetyl, the key enzyme of oxidative metabolism of long chain fatty acids, ie : the carnityl acetyl transferase, inside mitochondria, is due to the gradual accumulation in time of secondary oxidative damage to this mitochondrial lipid peroxidation.

The decrease of the energy return resulting, explains cellular aging because the cellular processes affected by this decline in energy production are essentially : the processes of detoxification, repair systems, the DNA replication, regulation of osmotic balance.

That is in fact, all the cellular metabolism which is affected by this decline in production of ATP.

It seemed that by increasing the supply of substrate, ie acetyl L cartinine, we restore the enzymatic reaction rate and by the same, the mitochondrial energy efficiency.

Besides, by adding a powerful antioxidant, such as the R. alpha lipoic acid, penetrating easily into the mitochondrias, we facilitate neutralizing free radicals but also is opposed to any increase thereof, which may result from the increased activity of the enzyme acetyl-carnityl transferase.

ROLE OF DIFFERENT COMPONENTS :

- The L carnitine

It is necessary for the penetration inside the mitochondria of long-chain fatty acids where they are beta-oxidized and where there is, thus, ATP production. The L-carnitine is also necessary to remove the fatty acids of short and medium chain.

The carnitine-acetyl transferase (CAT) catalyzes the reversible reaction between, on one hand, acetylcoenzyme A (CoA) and carnitine and, secondly, between the carnitine-acetyl and the CoA.

The essential function of CAT is to regenerate the CoA, which allows the beta oxidation to occur, and facilitate transport of acetyl residues into the mitochondria for oxidation and therefore the production of ATP from fatty acids.

Many experimental results have clearly shown that acetyl L-carnitine improves the metabolism of many tissues most likely due to the increased transfer of fatty acids and electrons through the mitochondria.

Also as a donor of acetyl group, acetyl L carnitine may participate in the increased of synthesis of acetylcholine.

He has been shown, that lower levels of acetyl choline in certain brain regions are associated with cognitive dysfunction including Alzheimer disease.

- The R Alpha lipoic acid

It is the coenzyme of the pyruvate dehydrogenase and of alpha ketoglutarate dehydrogenase in mitochondria. It is therefore necessary for the production of ATP from carbohydrates.

Dihydro-lipoic acid, the reduced form of lipoic acid is a powerful antioxidant which allows to recycle other antioxidants such as vitamins C and E and increased levels of intracellular glutathione, which is vital for the neuronal function.

Besides its role of scavenger of free radicals, the alpha lipoic acid is a very effective chelator, among others metals of transition of copper and iron, thereby reducing the catalytic activity of these metals in generative reactions of free radicals.

The accumulation of copper, and iron with aging may also be a cause of increased membrane lipid peroxidation.

In conclusion we can say that dihydro-lipoic acid may act : as a direct antioxidant, can increase the antioxidant power of other antioxidants such as vit C and E in both phases aqueous and hydrophobic at the level of cellular membranes and mitochondrials and, because of its chelating power of heavy metals, can oppose to the reactions oxidation catalyzed by these metals.

- Uncaria Tomentosa

Uncaria tomentosa could have a beneficial effect on memory in case of amnesia caused by cholinergic dysfunction.

The alkaloids seem to increase the central cholinergic transmission by increasing levels of acetylcholine or by affecting the dopamine system who can increase the cholinergic function.

Another constituent, Uncarina E, could also affect the glutamateric system who play a significant role in memory.

A study conducted by Professor Snow (Washington) showed that the deposition of beta-amyloid plaques in the brains of rats are significantly hampered by Uncaria tomentosa.

In the Alzheimer's disease, the amyloid-beta protein settles by patches in the brain while the brain cells form clusters stringy.

The researches in men are ongoing. The dose used in this indication is 60 mgr extract exempt of oxindole tetracyclic alkaloid in three doses per day.

- The Gingko Biloba

Ginkgo biloba at a dose of 120 mg to 240 mg per day of standardized ginkgo extract EGb 761 showed its effectiveness.

Several studies, conducted double-blind and controlled, showed that the use of extracts of ginkgo (or GBE for "Ginkgo biloba extract") can exert a modest, but significant, especially early in the disease.

In more severe cases, taking ginkgo would at least stabilize the condition or to slow the progression of the disease..

The efficiency of Ginkgo has also been compared to that of commonly used drugs (tacrine, donepezil, etc.).

The effect of ginkgo may take six to eight weeks before it appears.

Its action multifactorial justifies effectiveness in Alzheimer's disease. It affects the whole cascade of membrane disorders related to ischemia. Antiradical action, anti-inflammatory action, action on the microcirculation.
